All of the documents listed below are required in order for applications to be accepted.
All documents must contain the student's name and/or student number.
- The completed application form submitted online
- A resume or curriculum vitae
- Two references that can speak to one of the following:
- applicant’s demonstrated perseverance through mental health struggles, or
- applicant’s potential to be an outstanding participant in the community
